Workshop Description:
Transition planning is required for all students leaving the special education system, but for youth with social and behavioral challenges, including those who have experienced trauma, the services and linkages can be lifesaving. These youth need to develop core skills to become self-directed, learn their rights, and connect with community-based programs and services that can be a lifeline to success after graduation and beyond.
Attendees will learn:
In this two-hour program designed for Child Study Team members, school leaders, transition coordinators, and parents, a multi-disciplinary team of experts will discuss:
- Common challenges in transition planning for youth with social and behavioral challenges.
- Elements of effective, individualized transition services and supports for youth with mental health challenges and those who have experienced trauma.
- How to create and maintain linkages to adult services and supports.
- Effective and appropriate post-secondary resources for housing, employment and higher education.
- ADA Accommodations as they apply to those with mental health-related disabilities.
